You should provide the following documentation:

Any document prepared by the service member’s military branch or the Department of Defense (DoD)

Exclusions: DoD Education Loan Repayment Annual Application or DD Form 2475

A signed letter from the service member's Commanding or Personnel Officer that includes the information from Section 2 of the enclosed notice

OR the enclosed notice completed by the service member's Commanding or Personnel Officer.
